This session will help providers 1) recognize the challenges and barriers to effective communication regarding Advance Care Directives and End of Life Care, 2) learn and practice the requisite skills to confidently initiate these conversations with patients, and 3) know the available resources for both providers and patients that can help facilitate these conversations.
